I recently came across this verb in Ruth 1:3. My Bible software shows the root as mem-vav-tav. In this verse, though, it is parsed as a qal imperfect 3ms with a vav conversive, and the parsed form loses the vav in the middle.
BDB lists Gen. 42:38 as a reference for the qal perfect. Here Jacob is telling his sons that Joseph "is dead" or "he has died" so don't let me lose another.
The lexical form is, as you say, mem-vav-tav, and I always assumed that the lexical form would be the Qal perfect 3ms. Now I know this assumption is incorrect.
